| Duties: | POSITION SUMMARY
The role of the Home Services Representative is to assist in coordinating home health services and equipment. This position will establish and maintain relationships within the community through communication to referral sources such as extended care facilities, rehabilitation facilities, assisted livings and physician offices.
RESPONSIBILITIES:
•Complete all tasks related to acceptance and processing of referral from all sources.
•Educate patient/family and referral source regarding Covenant VNA services, equipment and processes.
•Validate documentation to meet compliance of payers and other regulatory requirements.
•Educate referral source on regulatory requirements related to referrals.
•May visit referral source for post admission documentation needs.
•Responsible for obtaining referral documents which may include visiting the referral source including but not limited to, extended care facilities, rehabilitation facilities, assisted livings and physician offices.
•Participates in staff education and conferences.
•Demonstrates critical thinking for process improvement and communicates opportunities of improvement to direct supervisor.
•Stays current with regulatory requirements.
•Participates in statistical reporting as required.
•Contributes to organization success targets for patient satisfaction and net operating margin by providing excellent customer service for performance.
•Maintains positive relationships with referral sources.
•Represents Covenant VNA with professionalism in communication, attitude and appearance.
•Other duties as assigned. |
| Experience Required: | EDUCATION/EXPERIENCE REQUIREMENTS
•Associate Degree in Health-Related Field or 3 years in healthcare/home care experience.
•Sales or marketing experience preferred. |
